Geisinger opens senior-focused 65 Forward Health Center
REEDSVILLE — Geisinger Medical Clinic Reedsville has opened its 65 Forward Health Center, rounding out care offerings at the facility that launched last year with traditional primary care, laboratory and radiology services.
The clinic opened its on-site pharmacy in September, and 65 Forward, Geisinger’s senior-focused model of primary care, completes programming available to the community.
Geisinger 65 Forward Health Center locations offer people age 65+ same-day appointments and more 1:1 time with doctors to help manage chronic conditions. They also provide:
• Strength and balance training
• Patient-tailored exercise programs to meet cardiovascular, strength, stability and flexibility goals
• Maintenance exercise following physical or occupational therapy programs
• Group classes
• Social activities to help combat senior isolation
• Health education and fall prevention
• Cane and walker fittings
Compared to a similar population cared for in a traditional primary care model, 65 Forward patients have had about 15% fewer hospital admissions and about 40% fewer emergency room visits.
Dominic DeMatteo, D.O, is a board-certified family medicine physician and the first doctor to join 65 Forward Reedsville.
“I’m looking forward to practicing in a model where I can spend more time with my patients to help them manage their total health and maintain their wellness,” said DeMatteo.
